Integrated Care and Case Management Worker, Housing, Mid West

Location: Limerick and Clare

Contract: Full time, 1 year Fixed Term

The purpose of this role is to work as part of the Focus Ireland Housing Service to provide integrated care, brief interventions and case management support to individuals and families in Emergency Accommodation across Limerick and Clare. The role will deliver structured, client centred support populations experiencing homelessness, with a focus on brief interventions, health stabilisation, emotional and social support interventions.

What you will do:

Work within the overall objectives, policies, and procedures of Focus Ireland, ensuring adherence to best practice standards.
Provide case management support to families and individuals in emergency accommodation across Limerick and Clare.
Ensure all customers complete a Housing & Wellbeing Assessment and receive appropriate follow-up supports.
Improve access to primary care, mental health, addiction, social care, and other support services.
Strengthen interagency coordination between housing, health, and community services.
Create a safe, welcoming environment that respects customer choice, dignity, and self-determination.
Lead on assessments and support planning using the Care and Case Management model.
Accompany and support customers to appointments (e.g. medical, counselling, and other services) as appropriate.
Work collaboratively as part of a multidisciplinary team and contribute to service delivery.
Provide advice, information, and guidance to prevent homelessness or respond to those out of home.
Support customers to integrate into their local community and develop life skills, including access to education, training, and employment opportunities.
Maintain accurate case records, daily logs, and database updates (including PASS), while managing an appropriate caseload.
Participate in meetings, supervision, training, and policy development, while maintaining awareness of health, safety, and welfare risks.

What you will need:

A recognised third level qualification in social care, mental health nursing, psychology, addiction studies etc and/or be a CORU registered Social Care Worker.
Two years’ experience of working with families or single homeless people, substance users, people experiencing poor general and/or mental health and/or other socially marginalised groups.
Experience of working with customers at risk.
Experience of team working.
Report writing abilities.
Experience of working and liaising with other agencies, including both voluntary & statutory.
Full Driver’s License with access to a car.
Excellent communication, interpersonal and report writing skills. (Desirable)
Knowledge and understanding of issues affecting people with complex general & mental health and addiction needs. (Desirable)
